Our aim at Jubilee Sports Physiotherapy is to deliver the highest level of expert care and service to our patients. The following Terms and Conditions are there to help you understand how we operate our practice. Please do not hesitate to ask if you seek further clarification or if you have any questions about our services.
By commencing treatment, you are deemed to have acknowledged and have agreed to be bound by these Terms & Conditions.
REFERRALS
Private fee-paying patients can access our services without a referral. If a referral has been provided (eg. general practitioner, sports physician, orthopaedic surgeon), please ensure this is provided to the treating physiotherapist.
Clients who are eligible for Compulsory Third Party (CTP) or Workcover funding must provide a medical referral for physiotherapy treatment, as part of their claims process. A claim must be approved by the insurance company to seek reimbursement from them directly. Until a claim is accepted the patient remains responsible for all fees.
At your initial consultation, please present the following:
BILLING AND ACCOUNTS
It is expected that all fees are paid at the time of consultation. We accept cash, EFTPOS and Visa/Mastercard payments.
If you have private health insurance, please bring your membership card to enable an immediate rebate (via HICAPS) for your convenience.
NON-ATTENDANCE AND CANCELLATION
We reserve the right to charge a cancellation fee if you fail to attend an appointment without giving prior notice. In the case of cancellation within 24 hours of an appointment we reserve the right to charge 50% of the treatment cost. Please note that insurance companies may not pay these cancellation charges and you will therefore be invoiced directly.
I understand that if I am late to my appointment, I may only receive treatment up until my scheduled appointment time, unless the physiotherapist determines there is insufficient time to conduct adequate assessment and/or treatment.
CONFIDENTIALITY AND PRIVACY
Jubilee Sports Physiotherapy is concerned with the privacy and safety of our clients. We are committed to collecting and handling personal information lawfully and fairly and to taking all reasonable steps to protect and safeguard your personal information. We do not share any personal information with any third parties.
We may track where you go or what you read in our website to allow us to improve the content and functionality of our website for all visitors.
Where Jubilee Sports Physiotherapy uses personal information to market our services to clients and prospective clients, recipients can contact us at any time if they no longer wish to receive this marketing information.
